HMO D-SNP

The 蘑菇视频 (HMO D-SNP) plan is for people who have Medicare and Medicaid.

You鈥檒l still have all of the coverage provided by both Original Medicare and Medicaid, and you will receive these extra benefits at no extra cost:

  • Dental, vision and hearing coverage
  • Allowance for over-the-counter drugs and health products
  • Fully prepared home-delivered meals following a hospital stay
  • Transportation to and from your doctor鈥檚 office and/or pharmacy

Plus, as a 蘑菇视频 Member, you will get support from a locally-based care management team to help you get the care you need.

Am I Eligible?

蘑菇视频 (HMO D-SNP) is available to anyone who has both Medicare and Medicaid. This can include low-income seniors ages 65 and older and people with disabilities who are over the age of 21.

The Texas counties in the service areas covered by 蘑菇视频 Medicare Advantage D-SNPs are Austin, Brazoria, Chambers, Fort Bend, Galveston, Hardin, Harris, Jasper, Jefferson, Liberty, Matagorda, Montgomery, Newton, Orange, Polk, San Jacinto, Tyler, Walker, Waller, and Wharton.
Service Area

Low Income Subsidy (LIS)

If you qualify for extra help, Low Income Subsidy (LIS), to pay for your prescription drug costs, Medicare could also pay 75% or more of your Plan Premium, annual deductibles and co-insurance (if applicable).
